Arterial oxygen content is associated with PEEP volume in patients without pulmonary disease

Resumen

Introducción: en los pacientes con asistencia ventilatoria mecánica (AVM) el agregado de PEEP determina un volumen de gas (PEEPVol) variable que contribuye a mejorar el intercambio gaseoso, reducir el shunt intrapulmonar y mejorar la capacidad residual funcional, la cual se encuentra reducida por factores como la sedoanalgesia, relajación y el decúbito supino. Conseguir un adecuado volumen pulmonar de fin de espiración podría mejorar el contenido arterial de oxígeno y la sobrevida. 

Objetivo: mensurar la relación del volumen de presión positiva al final de la espiración con el   contenido arterial de oxígeno y la capacidad residual funcional teórica.

Materiales y métodos: estudio prospectivo, descriptivo y correlacional que incluyó pacientes mayores de 18 años, con AVM ≤ 48 h y sin patología pulmonar, desde 1/11/14 al 1/11/16. Variables estudiadas: edad, sexo, talla, peso predicho (PBW), volumen tidal (VT), PEEP, PEEPVol (VTespirado – VTseteado), capacidad residual funcional teórica (CRFt) según fórmulas de la European Respiratory Society, y su relación con el PEEPVol (PEEPVol/CRFt), fracción inspirada de oxígeno (FiO2), hemoglobina (Hb) y contenido parcial de oxígeno arterial (PaO2), saturación de oxígeno (SpO2) y contenido arterial de oxígeno (CaO2) medido como 1,34 x Hb x SpO2 + (PaO2 x 0,0031), por cada 100 ml de sangre. Las variables se computaron luego de realizar una maniobra de capacidad inspiratoria y titulación de PEEP según curva de PEEP/compliance. PEEPVol se alcanzó colocando PEEP cero, reduciendo la frecuencia respiratoria a 5 ciclos/minuto y computando el VT espirado en el siguiente ciclo. Los resultados se expresan como %, media, mediana, DS, correlación (r de Pearson) y valor p.

Resultados: se incluyeron 28 pacientes con AVM ≤ 48 h y sin patología pulmonar. La edad promedio fue 58,6 ± 15,81 (mediana 65); hombres 62,96%, mujeres 37,03%; talla: 168,1 ± 6,72 cm (mediana 168); PBW: 62,48 ± 7,89 kg (64,19 kg); VT: 452,5 ± 63,80 ml (480 ml). PEEP: 7,71 ± 2,27 cmH2O (8 cmH2O); PEEPVol: 499,96 ± 211,48 ml (443,5 ml); PEEPVol/CRFt: 14,07 ± 5,28% (14%); CRFt: 3,64 ± 0,77 l (4.07 l); FiO2: 0,39 ± 0,09 (0,4); PaO2: 111,14 ± 27,77 mmHg (105,05 mmHg); SpO2: 0,97 ± 0,02 (0,98); Hb: 10,85 ± 2,02 g/dL (10,65); CaO2: 13,98 ± 3,78 ml (14,28 ml); los índices de correlación de Pearson para las variables estudiadas en relación al CaO2 fueron: PEEP -0,15, PEEPVol 0,51, VT 0,02, FiO2 0,03, PaO2 0,04 y SpO2 0,30; para PEEPVol/CRFt ≤ 14 fue 0,61 (p 0.0001) y para PEEPVol/CRFt > 14 fue -0,53 (p 0.01). 

Conclusión: el CaO2 mostró correlación directa y positiva con PEEPVol, lo que mostró que es una variable dependiente del volumen y de los cambios en la morfología del parénquima pulmonar antes que de variables como la presión. El incremento de PEEPVol a valores cercanos al 14% de CRFt se relaciona con mejor CaO2, mientras que valores superiores muestran correlación negativa. Futuros trabajos deberían valorar volúmenes de PEEP en relación con la CRFt y determinar su utilidad como indicador/predictor del intercambio gaseoso y el contenido arterial de oxígeno.

Abstract

Introduction: In patients with assistance mechanical ventilatory (AVM), the addition of PEEP determines a variable gas volume (PEEPVol) that contributes to improving gas exchange, reducing intrapulmonary shunt, and improving functional residual capacity, which is reduced by factors such as sedoanalgesia, relaxation, and supine position. Achieving an adequate end-expiratory lung volume could improve arterial oxygen content and survival.

Objective: to measure the relationship of the positive pressure volume at the end of expiration with the arterial oxygen content and the theoretical functional residual capacity.

Materials and methods: prospective, descriptive and correlational study that included patients older than 18 years, with AVM ≤48hs and without pulmonary pathology, from 1/11/14 to 1/11/16. Variables studied: age, sex, height, predicted weight (PBW), tidal volume (VT), PEEP, PEEPVol (VTespirado - VTsetado), theoretical functional residual capacity (CRFt) according to the formulas of the European Respiratory Society, and their relationship with the PEEPVol (PEEPVol / CRFt), Inspired Oxygen Fraction (FiO2), Hemoglobin (Hb) and Partial Arterial Oxygen Content (PaO2), Oxygen Saturation (SpO2) and Arterial Oxygen Content (CaO2) measured as 1.34 x Hb x SpO2 + (PaO2 x 0.0031), for every 100ml of blood. The variables were computed after carrying out an inspiratory capacity maneuver and PEEP titration according to the PEEP/compliance curve. PEEPVol was reached by setting zero PEEP, reducing the respiratory rate to 5 cycles/minute and computing the expired VT in the next cycle. The results are expressed as: %, mean, median, SD, correlation (Pearson's r), and p-value.

Results: 28 patients with AVM ≤48hs and without pulmonary pathology were included. The mean age was 58.6 ± 15.81 (median 65); Men 62.96%, women 37.03%; Size: 168.1 ± 6.72cm (medium 168); PBW: 62.48 ± 7.89kg (64.19kg); VT: 452.5 ± 63.80ml (480ml). PEEP: 7.71 ± 2.27cmH2O (8cmH2O); PEEPVol: 499.96 ± 211.48ml (443.5ml); PEEPVol / CRFt: 14.07 ± 5.28% (14%); CRFt: 3.64 ± 0.77lts (4.07lts); FiO2: 0.39 ± 0.09 (0.4); PaO2: 111.14 ± 27.77mmHg (105.05mmHg); SpO2: 0.97 ± 0.02 (0.98); Hb: 10.85 ± 2.02g / dL (10.65); CaO2: 13.98 ± 3.78ml (14.28ml); Pearson's correlation indices for the variables studied in relation to CaO2 were: PEEP -0.15, PEEPVol 0.51, VT 0.02, FiO2 0.03, PaO2 0.04 and SpO2 0.30; for PEEPVol / CRFt ≤14 it was 0.61 (p 0.0001) and for PEEPVol / CRFt> 14 it was -0.53 (p 0.01).

Conclusion: CaO2 showed a direct and positive correlation with PEEPVol, which showed that it is a variable dependent on the volume and on changes in the morphology of the lung parenchyma rather than on variables such as pressure. The increase in PEEPVol to values close to 14% of CRFt is related to better CaO2, while higher values show negative correlation. Future work should assess PEEP volumes in relation to tCRF and determine its usefulness as an indicator / predictor of gas exchange and arterial oxygen content.
